A flowchart that shows the process of obtaining a free product from a government insurance company. The flowchart is divided into four sections each representing a different step in the process.
The first section is labeled "Prior Authorization Support" and shows that the company provides a free purchase of a product. The second section is titled "Commercial Insurance" and has a list of options such as "IRC OPT-IN" "Government Insurance" and "PA Approved". The third section is labeled "PA Denied". The fourth section has a description of the product which states that it is a free-to-pocket assistance program that provides free support for patients who meet income and clinical requirements. The fifth section has an explanation of the PA Denied program stating that the program is a "Patient Assistance Program" program and that it provides a "Free Product" program for the patient who meets their medical requirements.
At the bottom of the flowchart there is a note that states "A free product is initiated for a Government Insured Patient cannot seek reimbursement for the remainder of the year. Free product must be provided for remainder of this year."
