A risk assessment for abuse and addiction. It is divided into three sections.
The first section is titled "Risk Assessment for Abuse and Addiction" and has a blue header with the title "Lower Risk". Below the header there is a table with three columns. The first column lists the risks associated with abuse the second column lists moderate risk and the third column lists higher risk.
At the bottom of the table there are three bullet points that explain the different types of risk assessments and how they can be used to assess the severity of the situation. The risks are:
- For patients with a history of drug abuse psychiatric issues or serious abuse
- Frequent and stringent monitoring
- Evaluating for appropriate treatment
- Referral for assistance in treatment
There is also a section titled "Proper Patient Selection" which lists the proper patient selection criteria for the risk assessment.
